If your pet has gone missing, letting us know will allow us to help you recover your pet. When a pet owner reports to us that a pet has been lost/stolen, we can put an alert on your file that the pet is missing and create a lost/stolen report to fax to the veterinarians/shelters in the area the pet went missing. Filling out the form below will alert us that your pet is missing. Please include any new information that may have changed since your pet was microchipped.

Please note that all missing pet reports will be done during regular office hours of 7:30 am to 5:00 pm Mountain Time, Monday through Friday.
